Changing employer with 186 Direct Entry application in process - seeking advice

I'm currently waiting on a 186 Direct Entry visa application, which I submitted about four months ago in March. It's for a regional healthcare role, and based on current estimates, it seems I'm looking at a 12 to 20 month wait, with some trackers suggesting an outcome around February next year. The thing is, I've got an opportunity for career advancement that would involve changing employers. My big worry is how this might impact my ongoing PR application and whether it could set me back significantly, which is the last thing I want after waiting this long. So, I'm hoping for some insights on a few points. If I change jobs, would my new employer be able to sponsor me for another 186, or would it have to be a 482 visa? I understand I'd likely be paying application fees again either way. Also, given how long these 186 applications are taking, do you think it's just better to stick with my current employer until I get a decision? And lastly, if I were to move on from my current role before the generally understood two-year period, what kind of issues could that cause for my visa status? Any advice or experiences would be really helpful!
